BERK assisted the Confederated Tribe of Coos, Lower Umpqua, and Siuslaw Indians in completing a community needs assessment using the Center for Disease Control’s CHANGE Tool. The Tool is designed to identify and understand the status of health needs, to assist with prioritizing community needs and allocation of available resources, and to aid in development of population-based strategies to create a healthier environment.

BERK integrated the Tribe’s data with the CHANGE tool and prepared results for submission to the Northwest Portland Area Indian Health Board as well as summaries for use by the Tribal community.
